QUALIFICATIONS:
- Ability to transport self to meetings and other work sites.
- Maintain driver’s license and safe driving record.
- High school graduate or equivalent.
- The ability to treat a person with a disability with dignity and respect and build a rapport with them to make your work with them enjoyable.
- One year of experience working with the ID population.
GENERAL DUTIES: Cleans kitchen and ensures all Life Stages, Inc. license regulations have been followed.
- Administers medication and documents procedures according to procedure.
- Complete routine health maintenance; practicing universal safety precautions.
- Conducts required periodic safety drills.
- Maintains and ensures a clean, safe, and healthy environment for consumers.
- Ensures individual support plans are being carried out. Implements and provides assistance to group home consumers in individual care tasks.
- Modifies environment as needed to create positive behavior support for individuals. Observe for antecedent factors that influence behavior.
- Gives relevant input into consumer’s plan-of-care at quarterly and annual team plan meetings.
- Plans and assists with recreational opportunities, transporting clients when necessary.
- Purchases needed clothing and personal articles within budgetary limitations.
- Assures that other duties and responsibilities that may be assigned temporarily or permanently to the position are completed accurately, efficiently, and with a positive frame of mind.
- Assists with the interviewing process of applicants.
POLICY AND PROCEDURES: Is familiar with and knows how to access all agency and assigned division policies and procedures in order to comply.
- Follows all safety policies and procedures of the agency.
- Adheres to regulations regarding confidentiality and human rights.
- Assures that personal certification/licensure requirements are current.
COMMUNICATION/DOCUMENTATION: Assists in the development of ISP plans.
- Communicates effectively and as appropriate with a supervisor. Meets with a supervisor as scheduled for case review sessions.
- Attends mandatory monthly staff meetings for case discussions/brainstorming, program assessment/development/planning, in-service training, etc.
- Demonstrates the ability to master basic computer skills needed to perform current and ongoing system needs and requirements.
- Accurately completes all required documentation within time requirements; extended SALs (Service Activity Logs), and monthly data sheets.
- Completes all other required documents, such as incident reports, workers’ compensation claim forms, etc. within the specified time limits.
- Communicates effectively verbally.
- Writes descriptive progress notes with appropriate assessment and plan.
CLIENT/CO-WORKER RELATIONS: Demonstrates the ability to establish rapport and maintain effective professional relationships with consumers, families, and community resources.
- Has the ability to work effectively as a team member by providing constructive feedback.
- Maintains a positive relationship with supervisor and co-workers. Relays accurate, up-to-date information.
- Shares workload demand in the unit.
- Welcomes and participates in the training of new employees.
- Receive consumers/visitors to the facility politely and with respect.

Hours per week:

- Monday-Friday we need 2nd and 3rd shift workers. The hours are 2:00 pm-10:00 pm and 10:00 pm-8:00 am.
- Saturday and Sunday we need all shifts; Ist, 2nd, and 3rd. The hours are 8:00 am-2:00 pm, 2:00 pm-10:00 pm and 10:00 pm-8:00 am.
