What is Lifestages’ ID Wavier Residential Home?

Our residential group home is designed to meet the specific needs of adults with intellectual disabilities and adults with Autism. This home is beautifully furnished and features a spacious kitchen with dining area, 4 private bedrooms, 2 bathrooms, gathering areas, expansive basement, and outdoor spaces. We believe each resident and day support attendee is an extension of our family. We encourage residents to lead a lifestyle as active and social as their abilities allow. Supervised by our experienced staff, residents make meals in the kitchen, gather for games, crafts and more.

The purpose of this ID Wavier home is to provide support in the community rather than in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ID).

Targeted Population:

Individuals 18+ who are at developmental risk and/or individuals who have Intellectual Disability (ID). All individuals must:

(1) Meet the ICF/IID level of care criteria;

(2) Require waiver services within 30 days; and

(3) Be determined that community-based services under the waiver are the critical services that enable the individual to delay or avoid placement in an ICF/IID or promote exiting from either an ICF/IID or another institutional placement.

Admission Criteria:

Program is administered by the Department of Behavioral Health and Developmental Services (DBHDS) and the Department of Medical Assistance Services (DMAS). The individual must be eligible for Medicaid and meet screening criteria; the income limit is 30% of the SSI payment limit for one person. The individual must meet criteria for ICF/IID; and must have ID or be under age 6 at developmental risk.
